Parents
should take the following items to the school for registration: |

Child's Social
Security Card |
|
|
|
 |
Proof of Residence
- Examples include a rental agreement, housing contract,
electric bill, or gas bill; telephone and cable bills are not
acceptable. A driver's license will not be accepted. |
|
|
|
 |
Official Birth
Certificate - Official birth certificates may be obtained from
the county or state health department where the child was born.
Copies of birth certificates will not be accepted. |

|
|
 |
Immunization
records - Medical evidence of the following immunizations must
be submitted: |
|
|
|
|
- Diptheria-Tetanus-Pertussiss (DTP or DtaP)
Vaccine: 5 doses. If 4th dose was given on or after child's 4th
birthday, the series is complete.
- Polio Vaccine: 4 doses of inactivated/oral
polio vaccine. If the 3rd dose is given on or after child's birthday,
the series is complete.
- Measles Vaccine: 2 doses received at least
30 days apart: one dose on or after 1st birthday and a second dose
before enrolling in school(k-1) for the first time. (Commonly given as
MMR.)
- Rubella Vaccine and Mumps Vaccine: 1 dose
of each on or after 1st birthday. (Commonly given as MMR.)
- Haemophilus Influenza type b Vaccine(Hib):
1 dose on or after 1st birthday or the complete series. Hib is not
given if child is 5 years or older.
- Hepatitis B Vaccine(HBV): 3 doses
- Varicella: 1 dose for children born on or after April 1, 2001 without documented history of disease.
- Note: Some children may have
received Tetramune Vaccine which includes the DTP and Hib vaccines.

Health Assessment
Form - North Carolina law requires a kindergarten health
assessment (physical examination) before a child can enter
kindergarten. This health assessment shall be made no more than 12
months prior to the date of school entry. Proper forms are available
at Wake County schools, the Wake County Department of Health, and
area doctor's offices. An exam can be obtained at the health
department if you have no physician or health insurance. |

Important Notice:
Students entering kindergarten in the 2009-10 school must be 5 years of age on or before August 31, 2009, in accordance with North Carolina G.S. 115C-364
